More explanatory documentation regarding OUTPUT_URL=null

Additionally more explanatory documentation in default.conf
about proper usage of quoting for BACKUP_PROG_INCLUDE
and BACKUP_PROG_EXCLUDE which is crucial to get
the intended results.
